DPH needs participants to take 5-minute online chronic disease survey

District 4 Public Health is seeking community members to complete a short online survey to better understand individuals’ health needs as they relate to chronic diseases and chronic disease prevention through healthy lifestyles and behaviors.
The anonymous survey takes about 5 minutes or less to complete and is open to anyone 18 years of age and older who lives in Butts, Carroll, Coweta, Fayette, Heard, Henry, Lamar, Meriwether, Pike, Spalding, Troup, and Upson counties. The last day to complete the survey is Tuesday, June 24. To begin the survey, visit https://bit.ly/cdpp_survey
District 4’s Chronic Disease Prevention Program will use the survey responses to better understand the reasons, disparities, and experiences that cause poorer health statuses and outcomes in the 12 counties it serves. The survey will ask for participants to provide the name of the county in which they live.
The survey is an opportunity for community members to help shape the Chronic Disease Prevention Program’s ability over the next several years to support healthy behaviors, reduce health disparities, and achieve its community health goals in District 4’s
Strategic Plan and Community Health Improvement Plan, said Staci Bagley, District 4’s chronic disease prevention program manager.
“As we continue our focus on helping to prevent chronic diseases through healthy lifestyles, we know the best way to understand and respond to our community’s health needs is to hear directly from as many individuals as we can,” Bagley said. “I encourage everyone who reads this to complete the survey and even share the link with friends and family members. We want as many people as we can to have a voice.”
