Why do complaint forms ask for social security number?

I recently sent a letter to the city manager, city council, mayor, the county commissioner and the county manager, as well as Chief Lou Dekmar and Sheriff James Woodruff.
Recently I filed a complaint form on a LPD officer and deputy. The form is not user friendly in my opinion. President Donald Trump wants every state to send to the White House social security numbers, maiden name and marriage name on voter’s lists.
Forty plus states have no problem in doing this. I believe he says this is needed to control voter fraud that is suspected in some states. LPD and the Troup County Sheriff’s Office also need to be asked why their complaint forms are asking for social security numbers along with date of birth.
LaGrange and the county seems to me to be making it hard for people to file a complaint against law enforcement.
